Serves adults 18 or older.
First time patients must register and bring: Proof of income (W2 form/tax return OR 2 current pay stubs) Health insurance card (if applicable) Social security card Driver's license or Photo I.D. and Proof of Dependents (tax document or Social Security Card for any children). Bring any necessary paperwork from employer for work physicals.
By appointment Monday and Thursday: 9:00am-8:00pm Tuesday, Wednesday and Friday: 9:00am-5:00pm Same-day and limited walk-in appointments available. Walk-in clinic (acute problems and physicals only) two Saturdays each month starting at 8:30am (call or check website for which Saturdays.)

Phone for an appointment M-F: 8:30am-4:30pm or walk in during walk in hours (Uptown Office). Urgent needs may be referred to other organizations if unable to schedule. There may be a long wait on the phone. Uptown Office: Saturdays: Clients are advised to arrive early as patients are seen on a first-come, first-served basis.

Third-party payment accepted, including Medicaid and Medicare. Financial Counselors assess payment based on income for those without insurance. Sliding fee scale rates are applied based on income, scales goes to zero.

Description

Provides general medical services. Offers treatment of ailments including coughs, colds, flu and rashes. Provides routine physicals for school, foster care, or work. May be able to provide clients with a limited supply of some prescriptions. Will also help clients access and complete paperwork for patient assistance programs that provide prescriptions.
