Serves residents of Lorain County.

  • Medical or insurance card, if applicable.
  • If do not have insurance: Bring proof of current income, including income of any spouse. Proof of income includes pay stubs and benefit statements. The agency must have this information when you come in to determine the cost of your service.
Hours
Monday - Thursday: 8:00am to 2:00pm. Friday: 8:00am to 1:00pm.
