Application process

Download and complete registration packet. Return completed registration packet and required documentation by email or in person during office hours. After submission, Child Focus will contact an individual to schedule an intake assessment.

Fees

Vary by service provided. Sliding fee scale available for eligible patients.

Offers evaluations for children and youth to determine the need for psychiatric medication to manage mental health related symptoms. Also offers medication management, including follow-up to evaluate the effectiveness of the medication.
