Eligibility

Adults 55 or older, or 54 or younger with a physical disability.

Required documents

Will need the following information: photo I.D., date of birth and Social security number for self and spouse, State and ZIP code, Veteran status for self and spouse, length of time in current residence, a list of the prescription medications currently taking, including dosages, current income from all sources for self, spouse and others in the household, and an estimate of current bills such as mortgage/rent, utilities, medical bills etc. Will be advised if other documents/information are needed.

Description

Offers one-on-one assistance to help individuals determine which private, public, state and local benefits they may be eligible to receive. Includes benefits such as Medicaid, Medicare Part-D, SSI, SSD, SNAP (food stamps), HEAP, Golden Buckeye Card, Homestead Exemption, prescription assistance programs, housing and others. Will also assist clients in obtaining and completing applications and will provide follow-up.
