Investigates and attempts to resolve all types of complaints about care provided to residents in licensed nursing homes and adult care facilities, and consumers of home care services. Ombudsman volunteers are present in nursing homes to speak with residents, observe conditions, and resolve simple complaints. The Ombudsman Program provides information on long-term care options, laws, and regulations.

Plans, funds, and coordinates services for older persons and persons with disabilities in Portage, Stark, Summit, and Wayne counties to live with dignity and choices in their homes and communities for as long as possible. Administers PASSPORT, MyCare Ohio, The Assisted Living Waiver, The Ohio Home Care Waiver, Care Coordination, Elder Rights, Family Caregiver Support Program, Nursing Home Placement Assistance Program, and other services designed to help older adults and individuals with functional limitations maintain independence. Provides funds for services administered by other community agencies including client finding, specialized information and referral, transportation, home-delivered meals, medical treatment, personal care, respite care, safety monitoring, health assessments, congregate meals, recreational activities, and adult day care.
