Phone for information M-F: 8:00am-5:00pm. For intake/appointments, may phone 24 hrs a day, 7 days a week. For intake, a written referral from the child's primary care physician is required. Medical records apply to patients.
Fee
Third-party payment accepted, including Medicaid and Medicare. Emergency visits are treated immediately regardless of insurance status. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis.
