Visit website for more information and to download the application. Will need to mail application with all required documents. Documents must be either originals or certified copies by the issuing agency. Does not accept photocopies or notarized copies of documents. Any documents mailed will be returned along with a receipt. Will have to wait approximately 7 days until it comes in the mail: no printouts will be given. Born outside the hospital: will first need a birth certificate and then can apply for the social security number at a local branch, can take up to 3 months. May phone for more information or to schedule an office appointment.

Administers the provision of Social Security retirement, survivors and disability benefits to eligible persons, as well as Supplemental Security Income and some aspects of the Medicare program. Also administers the assignment of Social Security numbers, changes in records, and processing of reports of lost or stolen checks. Individual Social Security offices are responsible for applicants who live within their service area, and all services are carried out at the local level. Clients are encouraged to go to the office which serves their Zip Code, though this is not mandatory.
