Eligibility

Serves pregnant women and women who are up to one (1) year postpartum in Lorain County (excluding the City of Vermilion, which is served by Erie County Health Department).

Provides central intake to women who are pregnant or postpartum to assist in locating resources and coordinate assistance by way of Neighborhood Navigators in the client's preferred setting (home, agency and/or community setting). Performs a detailed screening to identify barriers, needs, and opportunities for women and their families and then a step-by-step approach to action is taken to each item of need. Applies for and/or enrolls individuals in various community programs that the individual qualifies for, such as WIC and Medicaid.
