A medical application must be submitted by an approved physician, who obtains this form from the agent. Parent or guardian should contact their child's primary care physician to see if they are agency approved. May also contact a public health nurse at the local health department, at the agency's office, or see web site to locate approved providers or for detailed information. Parent, managing physician, and local health department will receive a letter of approval or denial, with explanation.
Free. Family may in some cases be eligible for insurance premium reimbursement.

Description
Provides payment for ongoing services related to an eligible handicapping condition. Eligible services may include inpatient hospital days, surgery and anesthesia, prescription medications, medical supplies and equipment, special formula, physical, occupational, and speech therapy, community nutrition services, and other services related to the eligible handicapping condition. Families must apply their health insurance, if any, to the cost of services.
