Application process

Three step enrollment process:

  1. Contact to schedule a facility tour.
  2. Enrollment forms will be provided during tour.
  3. Intake meeting with the program director required. Participant and their support team (parent/guardian, caregiver, SSA, etc.) must attend meeting.

Completed enrollment forms (program application, master medical form, and participant application) may be submitted online, at the intake meeting, or sent to Client Services.

Fee

Vary by service. Financial assistance available for eligible participants. Visit website or contact for more information.

Eligibility

Open to individuals with disabilities of any age who have completed high school. Eligibility will be further assessed by organization.
