Application process

Three step enrollment process:

Contact to schedule a facility tour.
Enrollment forms will be provided during tour.
Intake meeting with the program director required. Participant and their support team (parent/guardian, caregiver, SSA, etc.) must attend meeting.

Completed enrollment forms (program application, master medical form, and participant application) may be submitted online, at the intake meeting, or sent to Client Services.

Description

Provides comprehensive daytime care outside of the home for adults with additional needs. Services include personal care, independent living skill-building, and recreational and enrichment activities.
