Serves individuals 21 or older who live in Belmont, Carroll, Coshocton, Guernsey, Harrison, Holmes, Jefferson, Muskingum, and Tuscarawas county that have been diagnosed with a severe and persistent mental illness (SPMI) or with a diagnosed chronic condition (DCC) and have an active or pending Medicaid application.
