Offers hospice services for persons who are terminally ill. Services for clients include pain management, nursing care, personal care, art therapy, social work services, music therapy, respite for caregivers, spiritual care, counseling, and assistance with medications. Provides supportive services for families, such as social work and support groups.

M-F: 8:00am-4:30pm; nurse on call 24 hrs a day, 7 days a week. Families may receive bereavement services and support up to one year after a patient dies.

Application process
An order for hospice care will need to be obtained from a physician, if client does not have order, the hospice can help the client obtain one. Phone for information M-F: 8:00am-4:30pm.
Fee
Fixed Fee; Third party payment accepted: Medicare; Medicaid; Private insurer; may be able to provide financial assistance to those in need, no one turned away because of inability to pay.
Eligibility
Age 18 and older who are terminally ill and have a life expectancy of 6 months or less. For in-home services must also live in the following: Berea, Brecksville, Broadview Heights, Brooklyn, Brook Park, Cleveland (West - mainly 44135, 44144, 44111); Fairview Park, Lakewood, Middleburg Heights, North Olmsted, Parma, Parma Heights. Rocky River, Strongsville.
