Phone for an appointment M-F: 8:00am-5:00pm, or leave a message 24 hours a day, 7 days a week by voicemail. A referral/prescription from individual's physician is usually required for insurance coverage.

Fixed fee. Third-party payment accepted, including worker's compensation, Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to recieve a payment plan on a case-by-case basis.

Description

Provides a range of services designed to help people overcome physical injuries or illnesses and return to or remain at work. Services include comprehensive evaluations of workplaces, as well as individual physical therapy, occupational therapy, and vocational rehabilitation with an emphasis on activities that build individual skills and stamina for success in post-injury employment. Vocational rehabilitation services include initial consultations, testing of aptitudes/interests, career counseling, job seeking skills training, and job placement.
