Non-Emergency Transportation Plan (NET) | Cuyahoga County Job And Family Services

Provides transportation services or mileage reimbursement for Medicaid recipients for non-emergency medical transportation for miles traveled to a Medicaid covered service when the managed care plan provider cannot provide service.

Application process

Phone to verify eligibility and to request an Appointment Verification Form. There are three phone numbers: 1) for those with appointments for chemotherapy, radiation, or dialysis; 2) for those who need less than five rides in a month; and one for this with five or more appointments in a month. An application form is sent to the client, who provides it to his/her doctor. Doctor completes the form and returns it to the program office, where it must arrive 10 working days before the appointment. Agency then either sends cab vouchers or makes arrangements with a cab company.

Eligibility

Cuyahoga County Medicaid recipients. Serves those on a Medicaid HMO who have reached their yearly limit of rides (if they have not yet reached their yearly limit, they must contact their Medicaid HMO). Also serves Medicaid recipients who are not on a Medicaid HMO (sometimes called "fee for service") and who are eligible for medical transportation assistance through Medicaid. Transportation is for medical appointments only. Must be ambulatory (able to get into and out of a cab without assistance).
