Application process
To receive assistance, please schedule an appointment by emailing [email protected] or calling 740-200-0689. If you leave a voicemail, please be sure to include your name and the best way to reach you. Someone from the Care Team will respond to your inquiry as soon as possible.
Adults served must bring a copy of an official Ohio ID, such as a driver's license or other official ID. Some programs may require additional documentation. Your Care Team representative will inform you if other documents are required.

Will provide basic provisions and individuals to resources based on their unique situation and specific needs through our Choice Pantry or Emergency Food Distribution Center.
We help by providing:
- Fresh and healthy food staples with options for those with special dietary needs.
- Personal care products.
- Household products.
- Delivery service available by referral for individuals who are homebound or unable to pick-up.
