Eligibility

Adults age 65 or older or who are considered legally blind or disabled, or persons of any age who receive SSI or SSD. Must live in Darke County and be a U.S. citizen. Financial eligibility takes into account both income and resources and is set at 75% Federal Poverty Level (FPL). (Anyone who already receives any dollar amount of SSI will automatically qualify for Medicaid without filing a separate application or meeting an asset test).

Required documents

Agency can look up most supporting documents through an electronic records search - the client will be notified if they need to supply any extra supporting documents. If necessary, client may be asked to provide one or more of the following: proof of age, medical proof of disability, proof of medical expenses, proof of income and assets. Applicants who are U.S. citizens and not receiving Medicare or SSI may have to show proof of citizenship. Primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of US citizenship those who do not have one of these will need TWO documents: a certified birth certificate, hospital birth record, or religious birth record made within 3 months of birth, PLUS an identity document, which in most cases is a government issued ID such as state ID or driver's license. They may also need documentation of name change for any reason. If client cannot afford the necessary documents, or documents are unavailable, agency worker will assist the client to obtain them or find other options. Services will NOT be denied or terminated because of this requirement, as long as client makes an effort to comply.

Application process

Clients may begin the application process, renew, and/or update their case information by calling the Eligibility Contact Center: M-F: 8:00am-4:00pm. May also apply, renew, and/or update case information through website anytime (client must have an email address to do this). No interview is needed. Agency can look up most supporting documents through an electronic records search - the client will be sent a checklist for any missing supporting documents. Once an eligibility determination is made, a notice of action (NOA) is mailed to the applicant. Applications are available and may be dropped off M-F: 8:00am-4:00pm. MCP-eligible clients must select and enroll in an MCP, or will be auto-enrolled randomly (plan can be changed with reason, or during open enrollment). Those with general questions about Medicaid, Medicaid Managed Care Plans, or who simply need to change their address/contact information can call the Medicaid Customer Service Hotline M-F: 7:00am-8:00pm; Sa: 8:00am-5:00pm.
