Walk-in. Submit electronic or hard copy application to LifeCare Alliance by June 9th, 2023. Application is on website; also available on-site June 20th at Carrie's Cafe (670 Harmon Avenue). Coupons will be distributed on a first come, first served basis. Hard copy applications will be accepted on the distribution date. However, if an application is submitted at distribution event, LifeCare Alliance cannot guarantee that you will receive your desired choice of coupons or a produce box. When the initial resources are exhausted, the applicants will be placed on a waiting list. Applicants can designate a person shopper (proxy) on their application. The proxy must come to the distribution with the application signed by the participant. The proxy must also bring identification. A person can be a proxy for a maximum of two individuals.
