- live in the United States
- be a United States citizen/national or otherwise lawfully present in the U.S.
- not currently be incarcerated

Application process
Application required. Applications may be completed online or by phone. Enrollment must be completed between November 1-January 15 or during a special enrollment period. Eligibility for special enrollment periods is based on changes in household, changes in residence, loss of health insurance, etc.
