Outpatient Clinic - Adults With Chronic Illnesses | Centers For Families And Children - The
Provides a community clinic for adults who need chronic disease management for illnesses such as diabetes, hypertension, asthma or obesity. May be able to provide clients with a limited supply of some prescriptions. Will also help clients access and complete paperwork for patient assistance programs that provide prescriptions. Established patients only, may receive referrals for on site specialty care.
