Phone M-F: 8:00am-5:00pm to make an appointment. Staff will review any possible barriers to receiving service (transportation, insurance) when making appointment. May also request a schedule of upcoming events. Leave a message if no one answers and call will be returned within two business days. An appointment is required and may be requested by phone or through the registration form link.

Third-party payment accepted, including Medicaid and Medicare. Those who have no health insurance must have household income at or below 400% of federal poverty guidelines to apply for financial help. Financial Counselors will screen uninsured for Marketplace insurance, Medicaid, or hospital charity eligibility. Patients who qualify for charity might not always have a 100% of bill covered. Those who have an outstanding bill may be able to receive a payment plan on a case-by-case basis. For patients with insurance: program does not cover out of pocket expenses such as co-pays, co-insurance, and/or deductibles.
