Eligibility
Must be a Licking County resident. Residents under the age of 60 must have income at or below 150% of the federal poverty level. *Insurance is not a factor in determining eligibility.
Required documents
If diabetic, letter from physician required stating diabetes is under control
Proof of financial status.

Fees
No charge to qualifying clients. For those clients who have medical insurance, the vision center will bill the insurance company first and CVI will pay the balance.
