Eligibility
Must be a Licking County resident. Residents under the age of 60 must have income at or below 150% of the federal poverty level. *Insurance is not a factor in determining eligibility.
Required documents
If diabetic, letter from physician required stating diabetes is under control Proof of financial status.
Hours
Mon-Fri 8am-5pm.

Call for information.
Fees
No charge to qualifying clients. For those clients who have medical insurance, the vision center will bill the insurance company first and CVI will pay the balance.

We help Licking County residents of all ages who cannot afford professional eye examinations and glasses. We work with professional eye care centers to ensure every county resident can see their best regardless of their ability to pay.
