- Student(s) immunization record.
- Residency Affidavit Form and two (2) copies of proof of residency (tax bill, mortgage, utility bill).
- Custody Form, if applicable.
- Student(s) most recent report card or transcript (an unofficial copy is acceptable).
- Copy of the Individual Education Program (IEP), if applicable.
