Individuals who are blind, disabled or are 65 years and older. Must be a US citizen, national or qualified alien. Must have limited income and resources. Must apply for any other cash assistance they may be eligible for. Other eligibility requirements do apply; please contact SSA for details.
Completed application. Will be advised about required documents. Generally: Social Security Card, Birth certificate, proof of citizenship or alien status, proof of income, proof of resources, proof of living arrangements, medical records and work history.
