Eligibility

- Serves families with children and/or adults with a life-long disability attributed to developmental and/or intellectual impairments. To receive assistance these disabilities must affect daily functioning in the following areas: capacity for independent living, economic self-sufficiency, learning, mobility, social skills, receptive and expressive language, self-care, and self-direction. (Such as children and/or adults with Autism, Down Syndrome, blindness, or Cystic Fibrosis.) - Must reside in Northeast Ohio, within two (2) hours or 120 miles of Akron. - Must demonstrate a need. - A parent or legal guardian of a child may apply for assistance once per the calendar year per child. A family may apply for up to two (2) family members per year. - Will be required to have a face-to-face interview in Stow. - Will not reimburse for any services, programs or equipment that has already been received.

Required documents

- Online application. - Proof of income: A copy of every adults most recent pay stub who contributes to this household income. And a copy of the applicant's most current Form 1040 Tax returns and schedules. - Proof of need. - Child's Individual Education Program (IEP) or an adult's Individual Service Plan (ISP). - If applying for services (such as therapy), a “letter of service” must come from the service provider outlining the service that will be received, the cost per session and the length of treatment. This letter must come on letterhead and have an original signature on it. Must not be a copy. For equipment, provide a copy of where this item may be purchased.

Awards funds quarterly on January 30th, April 30th, July 30th, and October 30th.

Visit the website at www.zanesinc.org/application. Fill out online application. Documents must be sent by regular mail. Does not accept scanned items or emailed items. Does not reimburse for any expenses or treatments that have already been received. All funds will be given directly to the service providers. Assistance on a case-by-case basis.

Provides limited funding assistance for necessary incidental treatment-related services, programs and equipment not covered by insurance, government agencies or other resources to families residing in Northeast Ohio who have children and/or adults with a life-long disability attributed to developmental and/or intellectual impairments. Funds expenses such as respite care, therapies, therapeutic camps, some medical expenses, assistive/adaptive equipment, and communicative devices. NOTE: Does not provide financial assistance for rent, other living expenses, or any modifications done to home, property, or vehicles.
