Other eligibility may apply, but generally: Veterans who are terminally ill (life expectancy 6 months or less); or have a life-limiting diagnosis and are in a state of decline. Serves Ashland, Ashtabula, Cuyahoga, Erie, Geauga, Huron, Knox, Lake, Lorain, Medina, Ottawa, Portage, Richland, Sandusky, Seneca and Summit counties, with outreach to Morrow, Stark and Wayne counties.

Phone 24 hours a day, 7 days a week for enrollment information and to schedule an in-home assessment. An assessment will be done to see if the person is appropriate for this program.

Fixed fee but no one is denied service because of an inability to pay. Third party payment accepted: Medicaid; Medicare; Private insurance. A Patient Financial Advocate is available to evaluate financial assistance options.

Description

Provides personalized hospice care to veterans and their loved ones. Offers a trained staff educated in post traumatic stress disorder (PTSD), remorse and regret related to military service, substance abuse and the unique needs of those who served in the military.
