Applicants must:

  • have a household income at or below 500% of the federal poverty level
  • have a valid Social Security number
  • be in active treatment for cancer or have a cancer treatment plan in place
  • have been diagnosed with a type of cancer covered by the foundation
  • be covered by private insurance, an employer-sponsored health plan, or Medicare
