Case/Care Management For Older Adults | Council On Aging Of Southwestern Ohio

Offers assessment and case management for older adults to assist them with remaining in their home as long as possible. Through case management services, participants have access to a variety of in-home services, such as assistance with chores, home modification/barrier removal, home delivered meals, accompaniment to and from medical appointments, personal care assistance, etc.

Medicaid funds are used to pay for services. Co-payment may be required. Co-payments are set on a sliding scale based on income, assets, and medical expenses. Contact for more information.

Eligibility

Open to residents of Hamilton, Butler, Clermont, Clinton and Warren Counties age 60 and older who require assistance with certain activities of daily living. Eligibility for specific services is determined by a care manager during an in-home assessment.
