IF AVAILABLE: Military service number, Social Security number, a copy of separation papers (DD-214 or WDAGO 53-55). Will need complete mailing address and daytime telephone number.

Assists Veterans obtain copies of their military records and decorations. Provides the necessary SF-180 form. Office also has the emergency phone numbers for all branches of the military in case a Veteran's records are needed for medical care. Assists Veterans in requesting a 'review of discharge' to modify their discharge status; for example, from 'general' to 'honorable'.
