Eligibility

Women from Cuyahoga County who are within 28 days of delivery. Serves those who are having their first child hoever, mothers who had previously miscarried, offered their child for adoption, aborted, or lost their children are eligible for this program. Fathers and other family members may participate in the visits in the mother wishes.

Description

Provides free bi-weekly in home visits by a registered nurse to women experiencing a first pregnancy and until their child is two years old. Mentorship is offered to help the client determine what goals they may have to to help them develop a plan to achieve them. Clients will also be offered information on infant safe sleep, nutrition, and parenting skills. Linkage to appriate community services will be provided.
