Application process

Provide Care Management of in home services to help Warren County residents age 60+ Must call for appointment with the Intake Coordinator 513-695-2271

Fee

Depends on income and medical expenses. Cost Sharing is reviewed with Care Management staff during enrollment process.

Eligibility

Must be 60 years of age or older living in a community setting (non institutional setting). Individual requesting services must meet physical eligibility and have some limitations that require mechanical or physical assistance to maintain independence.

Languages

Program contracts with Interpreter Services as needed.
