Refers individuals with aging-related or disability-related issues to the appropriate available services in the community to ensure that individuals' needs are fully met. Provides assistance in finding help for housing, transportation, in-home care, long-term care, and other health and wellness services including benefits screening. Also assists clients in applying for and being assessed for services such as PASSPORT.
