Call M-F: 8:30am-5:00pm to schedule an assessment to determine the level of service required. May have to leave a message that will be returned. Subsequent appointments will be scheduled after the assessment.
Fee
Accepts private payments, limited third party payment (Medicare and Medicaid). Accepts Medicaid-eligible pregnant women before they are receiving Medicaid. Will work with individual pregnant women who become clients to submit Medicaid applications.
Eligibility
Serves ages 18-49 who re residents of Cuyahoga, Lake, Summit, Geauga, Lorain, or Medina Counties. Accepts Medicaid-eligible pregnant women.
