Community Residence Program | Ravenwood Health

Provides a program for severely mentally disabled adults, most of whom have previously lived in residential treatment facilities, who have progressed to the point where they require less supervision and support and are able to live more independently in the community. Provides housing in various agency-owned an/or managed homes located throughout Geauga County. The residential services staff and community support staff provide assistance and support as needed to help ensure successful independent living.

Application process

Applicant, family, or case manager should phone to apply M-F: 8:00am-4:30pm. The residences are generally full; applicants should expect a waiting period. Agency will hold applications for one year, after that client will need to re-apply for service.

Fee

Participant pays 35% of income up to fair market rent.

Eligibility

Open to any Geauga County resident age 18 or over who has a history of serious mental illness, and who can now live in the community with less than full residential support. Some units have income requirements (generally 50% Area Median Income or less).
