Anyone who suspects that an adult may be in need of protective services should contact Adult Protective Services by phone with the following identifying information: - The adult’s name, age, race/ethnicity, and address. - Caregiver information. - Circumstances regarding the abuse, neglect, and/or exploitation. - The alleged perpetrator's access to the adult. - The adult's current condition and information regarding any evidence of previous abuse, neglect, and or exploitation.
