Must have a breast cancer diagnosis. Serves women, men, and their families in Northeast and Northwest Ohio. Applicants are eligible once in a 12 month cycle.
Must complete an "Application for Assistance" including all necessary signatures and required pathology report. The foundation cannot process incomplete applications. All signatures must be original and, therefore, stamps, photocopies, or initials will not be accepted.
The foundation does not have regular office hours. The Foundations' Board of Directors meet quarterly to review grant applications.
