Eligibility
• Must be within Lucas County • Must be 18 years or older • Can be single or have a family • Must be chronically homeless 9 months or longer with verification of homelessness. (Verification must be from a reliable source, i.e. doctor, case manager, another mental health agency on their letter head etc.) • Have a Disability (This is determined by the Housing Impairment Form, in which a diagnosis is needed to complete this document) • Must complete an SPDAT (Must score in permanent supportive housing to be in Housing First).
