Eligibility

- Serves uninsured individuals residing in Northeast Ohio at or below 200% of the Federal Poverty Guidelines. - Will not assist Medicaid or Medicare recipients. - Patients ages 17 and under must be accompanied by a parent or guardian with valid photo ID.

Required documents

Must bring photo ID. Must sign an affidavit noting monthly income and lack of insurance.

Tuesday and Thursday: 5:30pm to 8:30pm.

Provides free medical care for uninsured individuals. Treatment services include: - Sudden illness such as cough or cold, stomach pain, sore throat, rash, headache, ear ache, and so forth. - Minor injuries such as cuts, sprains, back pain, and possible broken bones. - Health screenings such as blood pressure and diabetes checks. - Initial treatment for tooth, eye injuries, or infection. - Referrals for emergency or follow-up care including mental health care. - Free or discounted prescriptions. - Spiritual and emotional support from a Christian perspective.
