Eligibility

Available for qualified parents or families of children being cared for in certified homes and licensed child care centers. Must be employed or attending an accredited school and income is below 145% of poverty level. Ongoing (maintaining continuously) eligibility thereafter is 300% of the Federal Poverty Guideliens. Required documents

Application, proof of address, driver's license/photo ID, proof of income, Social Security card, birth certificate, proof of employment, and a selected certified child care provider.
