Serves low-income individuals residing in the United States who are substance-free for at least (1) year and have a stable, independent living environment. Visit the website for other eligibility guidelines. Will not help with any past medical bills, including items or services that have already been ordered. Will NOT accept self-referrals or referrals by family/friends. Applications must be submitted from a referring professional.
Required documents
Completed application form. Attach a written vendor estimate and a letter from the applicant's physician describing his/her overall health and need for the requested item.
