- Application must requested online by and submitted by a referring professional such as school personnel, a counselor, a teacher, a nurse or health aid, clergy/pastor, human resources staff, senior center staff, hospital staff, social service caseworker, social service technicians, or staff and volunteers at human service agencies. - To request an application and guidelines, visit the website and click on "Apply For Help".

Provides limited medical care expense assistance for the cost of: - Adaptive Equipment. - Assistive technology (medically prescribed). - Bioness and walk-aid devices. - Compression garments. - Hearing aids. - Lift chairs. - Medical equipment. - Mobility equipment (wheelchairs, scooters, lifts, and so forth). - Orthopedic braces and shoes. - Prostheses. - Stairlifts. - Vehicle adaptations, hand controls (no van conversions). - Wheelchair ramps.
