Eligibility

- Must live in Jefferson County and be a United States citizen. - Serves individuals of any age who need long term care. - Financial eligibility is an income at 225% of the Federal Poverty Level and assets up to $2,000 for an individual and $3,000 for a couple. Those whose income is higher must create a Qualified Income Trust (QIT).

Required documents

- Proof of identity and age. - Social Security number. - Citizenship or legal alien status (green card). - Need for nursing home level of care. - Medicaid number if already in place. - Income, assets, and health insurance status. NOTES: - Some information can be cross-checked electronically, reducing the physical documents required. - Applicants who are U.S. citizens and NOT receiving Medicare or SSI will have to show proof of citizenship. Primary documents for this purpose are a valid or expired passport, certificate of naturalization, or certificate of U.S. citizenship. Those who do not have one of these will need TWO (2) documents: a certified birth certificate, hospital birth record, or religious birth record made within three (3) months of birth, PLUS an identity document, which in most cases is a government issued I.D. such as state I.D. or driver's license. They may also need documentation of name change for any reason. If client cannot afford the necessary documents, or documents are unavailable, agency worker will assist the client to obtain them or find other options. Services will not be denied or terminated because of this requirement, as long as client makes an effort to comply.

Application process

Call to schedule appointment. Initial application may also be submitted through website (once application is received, client will be contacted within five (5) business days to schedule an appointment). An intake interview is necessary in order to obtain service (Homebound individuals may conduct the interview by phone or send an authorized representative in their place. Call for details.). Clients who may be eligible for Medicare will be referred for enrollment. NOTE: Those who are waiting for disability determination by Social Security can receive their disability determination for Medicaid through the county. This process can take weeks to months.
