Eligibility
Serves Summit County and surrounding areas. Call to determine eligibility. Generally provides transportation within 15 miles of Fairlawn (44333). May transport or pickup further, on a case-by-case basis.
Required documents
Authorization from reimbursement source if applicable. Medicaid billing number or Social Security number and Date of Birth. Other needed information is a pickup address, drop off address with suite number or doctor name, appointment time, and estimated length of time at appointment.
Hours
  • By appointment only.
  • Generally available Monday through Friday from 7:00am to 4:30pm, and Saturday and Sunday based on availability.
  • Other hours may be possible.

Fees
  • Varies according to service. Accepts Medicaid, Care Source Medicaid HMO, United Health Care Medicaid HMO, private insurance, and private pay.
  • Sliding scale based on income for private pay clients.
  • Group and individual rates are $50 to $59 per pick up, per person plus $3.50 per mile.
  • Allows a companion, such as a friend, family member, or nurse, to ride along at no extra fee.
  • NOTE: Agency does not accept Medicare.
