Eligibility
A person must be registered with the Body Donation Program PRIOR to the time of death. Donors must be at least 18 years of age. The person calling to inform the university of the death will be asked a series of questions to determine if the body is suitable for donation. The university also will need assurance that the body has been released by a physician. If the death occurs at home, the individual's physician must be called immediately prior to contacting the Northeast Ohio Medical University. The body must ARRIVE at the University within 10 hours of death, or the donation must be deemed unacceptable.
Required documents
Fill in the necessary information and sign the forms in the presence of two witnesses who must also sign in the applicant's presence. These signatures do not need to be notarized. One (1) copy of the witness form, general information form, and the medical records release form should be returned for Northeast Ohio Medical University's records. One copy of the witness form should be given to the applicant's physician, one copy given to applicant's next-of-kin, and one should be kept for applicant's records. Upon receipt of the completed forms, the Department of Anatomy and Neurobiology will provide a wallet-sized card that the donor may carry.

Write or call the Department of Biomedical Sciences for more information or to be sent a set of "witness forms," general information form, and medical records release form.
Fees
There are no charges or financial obligations for survivors. The program covers the cost for transportation to Northeast Ohio Medical University, embalming, cremation, and burial in the Homeland Cemetery in Rootstown, Ohio. However, if the ashes are returned to the survivors, Northeast Ohio Medical University will not pay for any expenses incurred after the release of the remains. No body donation can be accepted if the donor dies outside a 75-mile radius of the University.
