Youth ages 17 through 20 in Ohio. Family reunification must have been attempted and has been unsuccessful. Must have had difficulties functioning in a work or educational setting. Must not be a danger to themselves or others and not actively using drugs or alcohol. Most participants are in county custody.
Health insurance cards, if applicable, or 3-4 pay stubs as verification of income. Would want to see most recent school and mental health records.
Someone is on call 24 hrs a day, 7 days a week.

Phone intake M-F: 9:00am-5:00pm.

Fixed fee; Medicaid; Paid by the county if in county custody.

Provides furnished apartments for young adults who have had difficulties functioning in a work or educational setting. Helps residents to build independent living skills through case management and by teaching them life skills.
