Application process

Call to make an appointment for immunizations. COVID-19 immunization accepts walk ins.

Fees

No charge for surveillance services. Through the Vaccine for Children (VFC) program, there are no vaccination fees for uninsured children ages birth-18. Insurance is billed for other patients. Contact the Nursing Office for a list of accepted insurance providers.
