Bring Medical Card if available and photo ID. For proof of income to be considered for sliding fee schedule, one of the following documents will be accepted: 1.) One month of pay stubs 2.) a tax return, W-2 form or 1099 form from the most recent tax year 3.) Social Security benefit verification letter.

Moore Family Practice Location: Mon-Thur 8am-4:30pm Friday 8am-2:30pm Walk-In Clinic Mon-Fri 8am-4:30pm Danville Medical & Counseling Location: Mon-Fri 8am-4:30pm Danville Dental Location Mon-Thu 8am-6:30pm Fri 8am-4:30pm. Closed for all federal holidays.

Call 740-399-8008 for an appointment. Medical services at Mount Vernon and Danville locations. Paperwork completed before appointment or onsite. Walk-ins and appointments accepted.

Accept Medicaid, Medicaid Managed Care Plans, Medicare, insurance plans. Patients that do not have insurance will be eligible for sliding fee scale with proof of income. Out-of-pocket expenses can be reduced depending on income and family size. Costs are significantly lower than going to ER or urgent care. Significant cost reduction for any Lab work and prescriptions. Regardless of payer source, NO ONE is turned away. Nominal fee for dental services is $40.
