Provides community-based care coordination services for expectant mothers and fathers and parents with children under 18 months of age. Community Health Workers identify social and medical needs, connect individuals to appropriate services and care, and provide other wraparound supports. An emphasis is placed on prenatal and postpartum care and education on reducing the risk of infant mortality.
