Serves individuals age 65 and over, eligible people with disabilities, and people of any age with permanent kidney failure.

Required documents

An online application. Other documents will be requested as needed.

Hours

  • Initial Enrollment Period: Three (3) months prior to the month of age 65 and three (3) months after the month of age 65.
  • Special Enrollment Period: Eight (8) months after the last month of current work under an employer group health plan (not Cobra).
  • Open Enrollment Period (every year): October 15th through December 7th.
