Call 419-562-5871 ext. 1209 to schedule an appointment. Please have your health insurance, Medicaid or Medicare information available when you call.
Fees
Accepts most major health insurances. No child is turned away for Vaccines for Children (VFC) vaccines if their family is unable to afford the shots.

Offers recommended immunizations and vaccinations for infants, children, teens and adults. Curbside services available for those with mobility issues. Also available to assist Crawford County residents that are medically homebound wanting to get vaccinated.
