Senior Supportive Services | Catholic Social Services

Provides case management support to help seniors remain independent in the community. Services are based on the needs of the individual, but could include: a needs assessment, assistance with applying to government programs (Social Security, HEAP, Medicaid, Medicare), assistance in managing finances, locating and applying for housing, assistance with medical appointment/prescription access, arranging transportation, end of life planning, making linkage and referral to other needed services, supportive counseling and problem solving.
